Complete and print the Online HSA Enrollment Form

(*HSA does not qualify Health Savings Account holders – please make sure that you meet the requirements)

If you are transferring a balance from your current HSA, download and complete the Combined Enrollment/Transfer Form. (The setup fee is waived when transferring all, or part, of the balance of your current HSA.)
